Healthcare form used to hold paper rhythm strips (also know as EKG strips) onto a carrier to allow them to be filed in a patient’s chart. The rhythm strips are secured to the form using adhesive covered with a throw-away liner. Typically hand-filled but laser compatible versions are available also.
